These Terms and Conditions govern your use of our website at optimize360now.com and our telehealth services, including hormone replacement therapy, peptide therapy, medical weight management, and related clinical services, as well as our SMS communication program. By accessing or using our website or services, you agree to be bound by these Terms and Conditions and our Privacy Policy, which is incorporated herein by reference.

1. Optimize 360 SMS Program

Optimize 360 operates an SMS messaging program to provide patients with appointment reminders, treatment updates, clinical follow-ups, prescription notifications, refill reminders, and health-related communications regarding your care.


  • Opt-Out Instructions: You can cancel SMS messages from Optimize 360 at any time. Text STOP to our number at any time. Upon sending STOP, we will confirm your unsubscribe status via SMS. You will no longer receive SMS messages from us after confirmation.
  • Rejoining Instructions: To resume SMS communications after opting out, contact our office at (866) 678-4360 or text START to re-enroll.
  • Help Instructions: If you experience issues with the messaging program, reply HELP for assistance, or contact us directly at Contactus@optimize360now.com or (866) 678-4360.
  • Carrier Liability Disclaimer: Carriers are not liable for delayed or undelivered messages.
  • Message and Data Rates: Message and data rates may apply for messages sent to you from us and to us from you. Message frequency varies based on your treatment plan and care activity. Contact your wireless provider for questions about your text or data plan.
  • Compliance: Our SMS program is maintained in compliance with the Telephone Consumer Protection Act (TCPA), applicable FCC regulations, and all applicable state and federal telecommunications laws.

2. Privacy Policy

Your privacy and the security of your protected health information (PHI) are important to us. Optimize 360 is a HIPAA-covered entity and complies with all applicable privacy laws. 3. Eligibility

To use our services, you must be at least 18 years of age. By using our website or services, you represent and warrant that you meet this eligibility requirement. Optimize 360 reserves the right to refuse service to any individual who does not meet eligibility criteria.

4. Geographic Restriction — Indiana Residents Only

Optimize 360 is a telehealth practice licensed and operating exclusively in the State of Indiana. Our prescribing physician holds an active Indiana medical license only. Clinical services, consultations, and prescription services are available solely to patients who are physically located in Indiana at the time of their appointment.


By proceeding with our intake process, scheduling a consultation, or receiving services from Optimize 360, you confirm that you are physically located in the State of Indiana. We are unable to provide clinical services, prescribe medications, or establish a patient relationship with individuals located outside of Indiana. If you are not located in Indiana, please seek care from a provider licensed in your state.

5. Medical Disclaimer

Optimize 360 provides telehealth hormone replacement therapy, peptide therapy, medical weight management, and related clinical services that require medical evaluation by a licensed physician. Our services are provided by or under the supervision of a physician licensed in the State of Indiana.


Our services are not intended to replace the advice of your primary care physician or other qualified healthcare providers. Always seek the advice of your physician or other qualified healthcare provider with any questions you may have regarding a medical condition. Never disregard professional medical advice or delay seeking it because of something you have read on our website or received through our communications.


Optimize 360 does not serve as your primary care provider. Patients are required to maintain regular care with a primary care physician for conditions outside the scope of our specialized practice.

6. Acceptable Use

By using our website and services, you agree to use them for lawful purposes only. You agree not to:

  • Use our services for any illegal or unauthorized purpose
  • Violate any applicable federal, state, or local laws or regulations
  • Attempt to gain unauthorized access to our systems, platforms, or patient data
  • Provide false, inaccurate, or misleading information in connection with your care or intake process
  • Use our website content, trademarks, or materials without prior written consent
  • Interfere with or disrupt the integrity or performance of our website or services


You acknowledge that providing false or incomplete information on your intake forms or during your clinical care may result in inappropriate treatment, and you assume responsibility for any consequences arising from such misrepresentation.

7. Payment and Billing

Optimize 360 operates as a direct-pay, self-pay telehealth practice. We do not accept or file claims with any health insurance carrier. All payments for services are due at the time of service as specified during the intake and scheduling process.


Current fee structure:

  • Initial consultation with new labs: $365.00 (covers lab orders, record creation, and follow-up physician visit)
  • Initial consultation with existing labs (within 6 months): $125.00
  • Prescription costs are charged separately following your follow-up consultation and prior to medication shipment
  • Missed appointment fee: $95.00 (for appointments not cancelled at least 24 hours in advance)
  • Finance charge: $20.00 applied to balances unpaid 60 days after first statement


You agree to provide current, complete, and accurate billing and payment information. All payments must be made prior to lab orders being submitted and prior to prescription medications being dispensed or shipped. Fees are subject to change with notice.

8. Refunds and Cancellations

Due to the nature of our clinical services, we do not offer refunds once services have been rendered, lab orders have been submitted, or prescriptions have been dispensed. If you need to cancel or reschedule a telehealth appointment, please notify us at least 24 hours in advance via phone at (866) 678-4360, text at 765-222-9990, or email at Contactus@optimize360now.com.


Cancellations received less than 24 hours before a scheduled appointment may be subject to the missed appointment fee described in Section 7. Accounts with unpaid balances remaining after 60 days may be referred to collections with an additional 10% collections fee applied, and continued service may be declined.

9. Limitation of Liability

To the fullest extent permitted by applicable law, Optimize 360 LLC, its owners, officers, employees, agents, and affiliated providers shall not be liable for any direct, indirect, incidental, consequential, special, or punitive damages arising from or related to your use of our website or services, including but not limited to:

  • Errors or omissions in website content or communications
  • Interruptions in service, technical failures, or platform downtime
  • Loss of data or unauthorized access resulting from circumstances beyond our reasonable control
  • Outcomes of medical treatment, including adverse reactions or therapeutic results, where care was provided in accordance with applicable standards


Nothing in these Terms and Conditions limits our liability for gross negligence, willful misconduct, or as otherwise prohibited by law. This limitation of liability does not affect your rights as a patient under applicable healthcare law.

10. Intellectual Property

All content, text, graphics, logos, images, and trademarks on the Optimize 360 website are the property of Optimize 360 LLC or its licensors and are protected by applicable intellectual property laws. You may not use, reproduce, modify, distribute, or display any content from our website without our prior written consent.

11. Telehealth Services

By scheduling and participating in telehealth consultations with Optimize 360, you acknowledge and agree that:

  • Telehealth services are provided via electronic communication technologies and may involve inherent technical limitations
  • You have the right to withdraw consent for telehealth services at any time without affecting your right to future in-person care (where applicable)
  • You are responsible for providing a private, secure environment during telehealth consultations
  • The information transmitted during telehealth sessions is subject to the same HIPAA protections as in-person care
  • No results can be guaranteed or assured from telehealth services

12. Governing Law

These Terms and Conditions are governed by and construed in accordance with the laws of the State of Indiana, without regard to its conflict of law principles. Any disputes arising under or related to these Terms and Conditions shall be resolved in the courts of Delaware County, Indiana, or as otherwise agreed by the parties.

13. Changes to These Terms

Optimize 360 reserves the right to modify these Terms and Conditions at any time. Material changes will be reflected by updating the Last Updated date at the top of this page. Your continued use of our website or services after any modification constitutes your acceptance of the updated Terms and Conditions. We encourage you to review these Terms periodically.
